For your convenience, you can click on this PTOSI Prescription Form, print it and have your referring physician fill it out during your visit.
Please call our office for assistance with the referral process. Referrals can be prescribed by a physician, physician’s assistant, nurse practitioner, podiatrist, dentist, case manager or QRC. Some insurance providers will authorize services without a healthcare provider’s prescription. We are providers for all major health plans and will be happy to help with your questions regarding your individual insurance coverage.
